There's no magic bullet when it comes to keyword density. Just repeat your keyword only as often as is necessary to properly explain what you are trying to say. Google only really has a problem when a keyword is used in an unnaturally sounding way in order to manipulate search results.

Over stuffing time has gone now. If your do over stuff or more repetition in Title and body of content you might be penalized. Time has changed. You better use relevant synonym in the content, in title and also in meta valued. As Google is co-relation data so your content might come in user query if it has some variation.

You May Repeat You keyword two times in your Title,but not the same one,Also in your article you may put as many keyword ,but they must be unique one,you can repeat same keyword but I don't recommend that you use it more than 3-5 times and not all with anchor text.

Like you are use keyword "free money",then this keyword are repeated lot of time in the article.so you can repeat them but make sure that you have only 1-2 anchor text in your article.(in case of spam article submission,you can use as many as you want)

Just don't use your keyword in an awkward way. Only use keywords in your article as it makes sense to do so. Google hates anything that looks forced, when it comes to keyword insertion.

I guess being redundant with the keyword will not only annoy the readers but also the search engine because that is considered keyword stuffing. For example your target keyword is syringe and it should appear not more than 5 times in the content. More likely you will include the keyword in the title of the web page so that makes it 6 appearances. And when you tag your content with keywords the target keyword will again appear. What if you have 20 of the target keyword in the content? You can probably answer that question.
