PHP forms with mySQL

1) My site has an existing mySQL database and table populated with a unique ID and the email address of purchaser.

2) Purchaser receives physical item, which has the unique ID. Owner goes to a specific page on the site which has two form fields (unique ID , email address). On submission, PHP checks if ID and email matches the table entries - if no then error message, if yes they are presented with a new page which has a textbox where they can enter their contact information and save. Also the purchaser can come back at a later date to update their contact information.

3) Another person who is not the purchaser but also has the unique ID can go to another page on the site which has a single form field (unique ID field). If it doesn't match then error page like above but if it does then they are presented with the contact information but in this case read only

4) Should be designed with security in mind ( eg recaptcha or similar, sql injection attack immunity etc). Note the site will have https

5) Rather than give you access to my site and database directly, I'd prefer that you just provide the code to achieve the above
