Need help to fix CSS / HTML & customise a mail form

I need someone who is experience in css / html to proof read and fix the outstanding errors on the site. And create a cutomised email form on the site, following the site css styles .

Requirements

For a clearer picture, outstanding errors on site can be seen in the attach files
What needs fixing would be the banner and top navigation.

Contractor would also be required to proofread and clean up the CSS.
Add css styles for tables.

I would also need a simple email form following the sites's css styles.
With required fields: Name, email, message
It has to be spam protected using hidden form fields.
